Custom-made or from a fetish jewelry store (most likely online). you may luck out and find some at a local porn shop. Places like Condoms To Go and Condom Sense just have cheap-o dress-up kits, but there's one porn shop near where I live that caters to a wider audience with plus-sized lingerie and nipple/clit jewelry. Your best bet would be to google it online, though.
