The "cylinder" rotates into a position where the barrel aligns with a round, largely by chance. Pulling the trigger releases the hammer inside the barrel. The bullet remains stationary, as a guidepost, while the cartridge exits the barrel at high speed. Expected accuracy is high because the lightweight brass projectiles are rifled as they scrape past the hammer they just shoved back down on their way out.
Spent bullets are conveniently ejected from the user's offhand side, or down, or up, or sometimes out the barrel itself, based on wherever the hell the bulk of the gun decides to spin when some fool pulls the trigger on this contraption.
