I think this is either Ultimate Form Kuuga before he learned to control it during the series (he pretty much could've been one of the bad guys during that time), or Kuuga Rising Ultimate from the All Rider vs. Daishocker Movie...

Actually, I think it's more that the artist didn't realize that it was supposed to be Kamen Rider Skull in that slot. All the riders on his chest are supposed to be movie exclusive riders.
