Ripped from the American Madoka Magica website circa 2013, during the promotion of the North American Madoka Rebellion theatrical release. The original .psd (yes!) is in the zip folder.

Due to the absurd resolution, .psd file format, and complete lack of any titles, credits, or promotional information, it's likely that this is an original asset sent from Shaft to the American distributor for the creation of promotional material for the movie, and subsequently accidentally hosted on the website's folder.
