I'm not sure if sexually_suggestive or suggestive_fluid is quite appropriate, as the ice cream isn't presented in any particularly sensual way. The art is also implying that she's just delivered a baby, so I tried adding some tags for that. The ice cream I guess is intended as her getting one for behaving, like a child might when they're at the doctor.
