From my Nasuverse knowledge, they are 2 entirely different Illyas. The one from the main series undergone many body modifications, to better perform in the Grail war in the years between the two events. The Einzberns were actually some massive cheaters all things said and done.

Correct lore, though the ones in control of Illya in that universe were the cheaters and also exceptionally cruel to her: creating this advantage required her wounds to be open again and again
