Its a shota because there's a little boy, with an older woman. Doesn't necessarily imply sex, but there's good chance there will be since this is Kloah we're talking about.

And yeah, he has a thumb. It isn't particularly emphasized, You'd have done well to notice that his LEFT hand isn't particularly polished either.
