Of course artists have seen faces of women before. Drawings do not need to look realistic, and often an artist will make a deliberate choice to deviate from realism for effect.

Real =/= Good.
The point of hentai as a whole is to see porn and sex in a different light than you would in real porn, it might be too cartooney for some of course, but I think details like the vacuum blowjob exist to make the girls look sluttier and give the whole piece an air of degradation and corruption
