There really IS something off about her face, actually. There's just something so hollow and creepy about her eyes, they just look like they're going to pierce into you and steal your soul. I think it's because she has the eyebrows of a neutral expression, coupled with the eyes and mouth of a happy one.
This is not what happiness looks like, this is what creepy and unsettling looks like.
That said, the artist has a proper grasp of how to draw facial features, they're not poorly drawn, it's just that the expression is rendered oddly, giving it the previously mentioned disturbing quality.
